Middelhoek studied at the Academy of Visual Arts and Technical Sciences in Rotterdam, where he was taught by Alexander van Maasdijk, among others. He initially worked as a carpenter, but devoted himself more and more to painting. He mainly painted landscapes, cityscapes and portraits. He adhered to a figurative style. In addition to being a painter, he was active as an etcher, draftsman and illustrator. In the thirties he made two plaques for his hometown of Brielle. Son Piet Middelhoek (1930-2004), like his father, became a painter. He was an uncle of the physicist Simon Middelhoek. 67506487
